1. What Hazel is

Hazel at Home is solely a matching and referral service. We do not provide home care, home health, medical care or nursing services. We connect families with independent, licensed local home care agencies ("Providers") that offer non-medical assistance.

2. Free for families

Hazel does not charge families. Providers pay Hazel a referral fee when a matched family begins services. Learn how we get paid.

3. Providers are independent

Providers are independent businesses and are not employees, agents or partners of Hazel. Any care, staffing, scheduling, billing or services are agreed between you and the Provider directly. Hazel is not responsible for care delivery, caregiver conduct, or the outcomes of your relationship with a Provider. You are responsible for evaluating any Provider before engaging them.

4. No medical advice

Nothing on this website is medical advice or a healthcare assessment. Rates and Medicare information are general and may not apply to your situation.

5. Rates and information

Published rates are typical averages. The actual rate is set by the Provider and may differ. We try to keep information accurate but make no guarantee.

6. Your information and contact consent

You agree to provide accurate information. By requesting a match, you agree to be contacted by telephone, SMS or email by the single matched Provider in your area, and by Hazel about your request. Our use of your information is described in our Privacy Policy.

7. Acceptable use

You agree not to misuse the site, submit false requests, or interfere with its operation.

8. Disclaimers

The site and service are provided "as is" without warranties of any kind, to the fullest extent permitted by law. We do not guarantee that a match will be available in your area.

9. Limitation of liability

To the fullest extent permitted by law, Hazel is not liable for indirect, incidental or consequential damages, or for any damages arising from services provided by a Provider.

10. Changes and governing law

We may update these Terms by posting a new version here. These Terms are governed by the laws of the United States and the state in which Hazel at Home is organized.
